Primary Required Documents
Valid Government-Issued ID (Original + Photocopy)
- Philippine Passport
- Driver’s License (LTO)
- UMID (Social Security System)
- National ID / ePhilID
- Professional License (PRC)
- PNP/AFP ID
- Bank ID (with photo and signature)
2×2 Passport Photo (2-3 copies required)
- Recent photo (taken within 6 months)
- White or light background
- Clear face, full frontal view
- No glasses covering eyes, no hats
- Natural hair style as normally worn
For First-Time Applicants
Barangay Clearance (Required – Original Copy)
- Cannot be photocopy – must be original
- Valid for 3-6 months
- Issued by your barangay hall
- Cost: ₱50-₱150
- Processing: 30 minutes to 1 hour
Proof of Purpose/Employment (Recommended)
- Employment letter from company
- Job offer or acceptance letter
- School certificate or diploma
- Scholarship acceptance letter
For Renewal Applicants
- Original expired or still-valid police clearance certificate
- Valid government-issued ID (original and photocopy)
- 2×2 passport photos (2-3 copies)
- Proof of payment (if paid online beforehand)
- Selfie with valid ID (for non-appearance renewal)
For Special Cases
If Name Changed:
- Marriage certificate (authenticated copy from PSA)
- Court order of name change (if legally changed)
- Divorce decree or annulment (if applicable)
If Address Changed:
- Utility bill in your name (electric, water, internet)
- Lease contract or rental agreement
- Property deed or title
